We offer personalised support to help you improve your health and wellbeing. We take the time to find out what matters to you, then suggest community based resources that may be of interest.
How can we help?
We can connect you with activities, support groups, learning, and volunteering opportunities. We can also connect you with services offering advice on benefits, housing, and other areas of support.
Our wellbeing connectors are knowledgeable about local resources and can help you find the right activities and support.
Who can benefit?
Our service is available to anyone registered with a GP practice within South-West, West, and North Cardiff.
