How do I get a referral?

You can get a referral via your GP surgery, or contact us directly by calling: 029 2000 3132.

What happens next?

A member of our team will contact you by phone within 7 days to discuss our services and arrange a meeting, either in person or over the phone.

Finding out what matters to you

We’ll begin by understanding how we can help and what support you would find useful. All of our support is optional, so the choice is entirely yours.

Seeking and suggesting options

We’ll help you discover local groups, activities, or services that interest you. If we can’t find something suitable, we may be able to assist in starting a new group or activity in your community. 

Follow-up appointment

We can arrange extra appointments to help you choose and access the right groups, activities, or services, and to tackle any obstacles, whether it's confidence or practical needs like transport.
